Contacting Dr. Rothschild
To contact Dr. Rothschild, please call (727) 786-8000. If he is not available, please leave a message and your call will be returned within one business day (typically by the end of the day). We currently do not communicate with patients via email.
If you cannot reach Dr. Rothschild immediately and you are calling with an emergent situation (including those in which a person is feeling out of control, unable to care for him/herself, or having serious thoughts of harming self or others) call 911 or go to your nearest emergency room. Alternatively, you can call the PEMS crisis line at (727) 791-3131.
Prescription Refills
If you are an established patients and need a refill of medication prior to your next appointment, please call the office to request a prescription refill. Please note that a refill request coming from you pharmacy is not sufficient as the office often receives numerous unneeded, automated requests from pharmacies.
Release Forms
Your private, protected information will not be released without your written consent on an appropriate release of information form. With a password provided by our office, the following forms can be printed and submitted to allow Dr. Rothschild to communicate with your other providers or family members. Each of these forms can be submitted in person, by fax or by mail.
To request treatment records from Dr. Rothschild or to release information to a family member please use the following links. Please note that you will first need to call the office and request a code to access these request forms.
